Additional copies of previously completed evaluation reports may be requested with the following form for $25 each report as long as it is within 4 years from the date of evaluation.
If you are unsure as to whether your report is within that four-year mark, you can contact us by email or phone. Please be prepared to provide your FCSA ID, date of birth, and name.
If you are needing your original documents returned to you, there is a $10 fee that includes USPS domestic tracking.
Any international address requires a $60 shipping fee as well.
